Anybody seen this before?
No, no photo, but today I examined what I thought were wind broken Phy viridis shoots, they were bent and nearly severed a few inches from the tip of healthy new shoots about 6' tall, no branching, fully wrapped in culm sheaths but where the 'break' was on 2-3 culms a branch bud had fired near the growing tip and the tip was quite bent/damaged. I've never seen this before, an upper branch bud firing so early and a thick one at that. Looks like a mutant shoot.

No apparent insect damage, even internally?
I had a few rubro shoots flop over about 2' off the ground this year -- not broken, just bent in a U so the tip was touching the ground. Not the same as what you describe I don't think, but similar?

Note that I've just seen more of those rubro shoots flop over. Is this not enough water? What's going on? (It's been in the 90's)

Not sure if this is your cause but I've noticed that culling culms while there are new shoots can lead to floppy new culms. Mine the issue is premature branch bud firing near the shoot tip.

Never seen that, but I have a Moso seedling that makes 90 degree turns on the top 1/3 of the shoot. It happes when shoots almost reach their final height and start leafing out.
Each year there are 5 to 10 shoots that end up looking that way.

I think mine might be due to a baby woodchuck climbing fresh culms.

Iv'e had this happen a few times from birds landing on new shoots.

Could be birds, I think that the branches fired too soon and created weak areas, some have since continued growing at angles.
